The COVID-19 pandemic heightened the awareness of healthcare disparities that have been affecting Chicago’s disadvantaged communities for decades. In this panel discussion led by stakeholders from the clinical, community, and designer perspective, we will address the importance of population health and how it impacted the COVID-19 response. We will discuss social determinants of health, especially as they relate to our Chicago communities, but also look at how healthcare systems are creating new solutions to combat health inequities. The audience will leave with a better understanding of population health and learn about opportunities of engagement that they can apply in their own practice.
